Meningitis Vaccination
Texas State law requires all students housed on campus to be vaccinated for meningitis. If accepted, I acknowledge I must bring proof of meningitis vaccination to participate in the program.

Travel
I acknowledge that travel to and from SURF at TAMUCC is limited to a maximum of $400 (return) regardless of my destination and/or mode of travel, that this is a one-time expense, and that any costs above this amount is my responsibility.
