Project Description: The Personnel Transaction Request Form is a very commonly and frequently used form by the administrative departments of the University. This form is to be filled out and approved by an employee’s administrative supervisor and department head and then reviewed by the Human Resources Department, the Budget Office and the Payroll Department for any change in the payroll information of an employee. This project involves the analysis, implementation and testing of a system that will facilitate the creation, approval, reviewing and tracking of this form online.
